Special Offer on GSI Memberships for Florida government Employees

Eligibility Verification: All applicants must provide proof of employment or residency (for family members) to qualify for this offer. Acceptable documents include government employee IDs, pay stubs, or other official documentation.
Offer Duration: This offer is valid until further notice. GSI reserves the right to modify or terminate the offer at any time without prior notice.
Membership Activation: Memberships must be activated within 30 days of approval to retain eligibility for the special rate.
Family Memberships: Family members (spouse and children) must reside at the same address as the government employee and provide proof of residency (e.g., utility bill or driver’s license).
Non-Transferable: Memberships and associated benefits are strictly non-transferable and apply only to the individual or family members registered.
Membership Access: The offer is only applicable for individual memberships; add-ons such as personal training, special classes, or other services will incur additional fees unless otherwise specified.
Cancellation Policy: A 30-day written notice is required for cancellation of the annual contract, even with no long-term commitment. No refunds for partial months.
Behavioral Expectations: All members are expected to follow GSI’s code of conduct, which includes respectful use of equipment and facilities and appropriate behavior towards staff and fellow members.
Age Restrictions: Children under 13 must be accompanied by an adult at all times.
Strength & Wellness Floor, may have stricter age requirements.

Payment & Fees:

Billing Terms:
Membership fees are billed monthly.
Any payment issues (e.g., declined transactions) must be resolved within 10 days to avoid membership suspension.
Additional Costs: While initiation fees are waived, optional services like court reservations, advanced classes, or locker rentals may carry separate charges.
Membership Freeze Policy: Members may freeze their account once per year for up to 3 months for medical or personal reasons, with written notice.

Liability & Access:
Liability Waiver: Members must sign a liability waiver before using the facilities.
Access Control: Membership cards are required for facility entry. Lost cards may incur a replacement fee.

Communication:
Promotions and Updates: GSI reserves the right to communicate promotional updates or changes to membership terms through email or other communication channels.
